Please fill this form if you were out for any day and submit it with your timecard
Vacation / Time off
All request must be submitted 30 days prior.
All though we will make every effort, there are no guarantees that vacation will be granted.
Please try to schedule your vacation in our slow time.
Any other time off request should be made ASAP before the requested Date.
REMEMBER
We don't pay parking fines for your vehicle or the company vehicle you are using. But, we do pay parking fees. So get a receipt put your name on it and turn it in to the office
NOTE:
Timecards must be submitted no later than 9pm on Sunday
